In Canada a hip replacement is an insured service, so nobody sends you a bill. What you pay with is time, and how much depends a lot on your province. This guide puts the latest CIHI wait data next to private prices in Canada and India's published 2026 figures in Canadian dollars, so you can see whether a trip to India is worth planning or whether the wait at home is shorter than you think.
How long Canadians actually wait for a new hip
CIHI's 2025 data tables show that, nationally, 69.2% of hip replacements happened within the 182-day benchmark. The median wait was 120 days, and one in ten patients waited longer than 321 days.4 The clock in these figures starts when you and your surgeon decide to go ahead, not at your GP referral, so the full journey from first appointment is longer.
| Province (2025) | Within 182 days | Median wait | 1 in 10 waited longer than |
|---|---|---|---|
| Ontario | 83.8% | 76 days | 230 days |
| Nova Scotia | 77% | 103 days | 289 days |

Paying privately in Canada
Private hip surgery exists in Canada but it isn't cheap, and provincial plans won't reimburse it. Windflower Surgical, which operates in Montreal, lists C$29,500 for one hip and C$44,500 for both.5 Surgency's 2026 guide puts private hip replacement at roughly C$21,000–C$40,000+, with Quebec at the lower end.3

Converted at C$1.41 per US dollar, the ECB reference rate for 24 September 2026, and rounded.8 Indian hospitals normally invoice international patients in US dollars, so the rate on the day you pay decides your real cost.
So the honest comparison for most Canadians isn't India against a free public hip. It's India against waiting, or India against a C$21,000–C$44,500 private operation at home. Against private surgery India can save real money. Against a public date a few months away, it often doesn't make sense.
What's in an Indian hip package, and what isn't
Inclusions swing widely. At the generous end, one facilitator folds in the surgical team's fees, the implant, drugs, a pump that helps prevent clots, physio, interpreting, airport runs and as many as five hotel nights.2 A hospital's own quote may be much leaner. Before comparing, request:
- An itemised quote in US dollars with exclusions listed.
- The implant brand, model and bearing surface. Ceramic-on-ceramic adds about 30–50% and robotic assistance about 20–25%, according to Bookimed.1
- The number of hospital nights and physio sessions.
- Who pays if you need a second operation or extra nights.
The trip: timing, cold weather and a long flight
Budget about two to three weeks in India.1 Hospital stays at the big chains are often three to five days, according to one facilitator, and the rest is physio and a wound check.2 From most of Canada the journey is long-haul with at least one connection. AAOS lists blood clots among the known risks after hip surgery and describes blood thinners, compression and early walking as the usual prevention, so get flight instructions in writing.9
Think about the season you'll come home to. AAOS warns that falls are a real hazard while you recover and that the risk of dislocation is highest in the first few months.9 Icy steps in January are a bad match for a six-week-old hip. If you can choose, a spring or summer return is kinder.
Follow-up back home
Global Affairs Canada's India travel page carries a medical tourism warning, noting that some Canadians have had serious complications after elective surgery abroad.10 Take it as a planning checklist rather than a reason to panic:
- Talk to your family doctor before you go and ask who will manage your follow-up and physiotherapy.
- Ask your province and any private insurer, in writing, what they'll cover if you need care for a complication after surgery abroad.
- Bring back the operation note, discharge summary, X-rays and implant details. CIHI's Canadian Joint Replacement Registry tracks hip and knee replacements across Canada, and a Canadian surgeon will want to know exactly what's in your hip if it ever needs revising.11
- Serious complications such as infection affect fewer than 2% of patients, AAOS says, but know which hospital you'd go to if you had one.9
Choosing a hospital: what to check
- JCI or NABH accreditation, confirmed on the accreditor's own directory, not on a brochure.12, 13
- The surgeon's volume: how many primary hips and revisions a year.
- Implant brand in writing before you book flights.
- A written discharge plan that says when you'll be cleared to fly.
We don't publish hospital rankings, paid or unpaid.
When staying in Canada is the better plan
If you live in a province with shorter waits and your date is close, if you have heart or clotting conditions, if you'd be alone during recovery, or if you need a complex revision, staying home is usually wiser. Private surgery in Canada costs more, but your follow-up stays with the same team.
Travel & visa: Canadian citizens need an e-Medical visa, and companions can apply for e-Medical Attendant visas. See our India medical visa guide for the hospital letter, timing and registration rules.14
